The short version

Corticosteroids, the tablets people call steroids, include prednisone. A Medrol Dosepak packs 21 tablets of methylprednisolone 4 mg across six labeled days. The Endocrine Society says a course shorter than 3 to 4 weeks can stop without a taper. After a longer course the step-down aims at a physiologic 4 to 6 mg prednisone day. Conversion cards print 5 mg prednisone against 4 mg methylprednisolone and 20 mg hydrocortisone.

Questions this tray answers

Quill asks: What is a Medrol dose pack? A Medrol Dosepak is 21 tablets of methylprednisolone 4 mg over six days. DailyMed's Day 1 row is two before breakfast, one after lunch, one after dinner, and two at bedtime.

Deke asks: How do you taper prednisone? The 2024 Endocrine Society guideline allows a stop after less than 3 to 4 weeks. Longer courses step down toward 4 to 6 mg prednisone a day once the disease is controlled.

Maren asks: Can you stop steroids suddenly? NIDDK says suddenly stopping a long corticosteroid course is the most common cause of tertiary adrenal insufficiency. Courses under 3 to 4 weeks can stop without a taper on the Endocrine Society guideline.

Sibley asks: What is steroid conversion? Vanderbilt prints prednisone 5 mg equal to methylprednisolone 4 mg, hydrocortisone 20 mg, and dexamethasone 0.75 mg.

Paxton asks: Is prednisone a steroid? Yes. Mayo Clinic calls it a corticosteroid, a cortisone-like steroid.

Ione asks: Is albuterol a steroid? No. Mayo files albuterol as a short-acting beta-agonist bronchodilator. ProAir and Ventolin are albuterol brands, not steroid inhalers.

Holt asks: How do you take prednisone? DailyMed recommends a single morning dose before 9 am, with food or milk. Multiple daily doses are spaced evenly.

Riva asks: Do you take prednisone with food? DailyMed says gastric irritation may fall if the tablet is taken with food or milk, or before, during, or right after a meal.

Clem asks: Can you buy prednisone over the counter? No. DailyMed lists the tablets as Rx only. Mayo says a doctor's prescription is required.

Enid asks: Can I buy prednisone online? A US fill still needs a prescription sent to a pharmacy licensed in the state that receives the tablet.

Joss asks: What are adrenal insufficiency symptoms? NIDDK lists fatigue, weakness, loss of appetite, weight loss, abdominal pain, nausea, and dizziness on standing. Crisis warnings include severe vomiting, confusion, and fainting.

Vale asks: Is prednisolone the same as prednisone? Vanderbilt prints both at 5 mg on the conversion card. The prednisolone eye-drop and Orapred leftover sits on the prednisolone page.

Nedra asks: What is a prednisone 20 mg tablet? One USP line is a peach round scored 20 mg tablet, debossed 5092 and V. Deltasone 20 is peach, scored, imprinted DELTASONE 20.

Kip asks: What are steroid side effects? Mayo’s prednisone-withdrawal list includes irritability, nausea, joint pain, and severe fatigue if a long course is cut too fast. Food or milk is on the tablet sheet for stomach irritation.

Opal asks: What is prednisone used for? Mayo lists inflammation, severe allergies, arthritis, asthma, lupus, bowel flares, and multiple-sclerosis flares among the uses.

Medrol Dosepak is 21 tablets of 4 mg

A Medrol Dosepak is a blister of 21 white elliptical 4 mg tablets, imprint MEDROL 4, NDC 0009-0056-04. DailyMed prints the six rows on the carton. Day 1 is two tablets before breakfast, one after lunch, one after dinner, and two at bedtime. Day 2 drops the second breakfast tablet. Day 3 keeps breakfast, lunch, dinner, and bedtime. Day 4 is breakfast, lunch, and bedtime. Day 5 is breakfast and bedtime. Day 6 is one tablet before breakfast. The same sheet says take all six Day 1 tablets the day the pack is filled, even if that fill is late; those six may be swallowed at once or split before bedtime. Range Medical's pack card writes the milligram totals as 24, 20, 16, 12, 8, then 4 mg, each tablet still 4 mg.

Endocrine Society: under 3 to 4 weeks, no taper

The 2024 Endocrine Society and European Society of Endocrinology joint guideline suggests no taper after glucocorticoid therapy shorter than 3 to 4 weeks, at any dose, because HPA suppression is a low concern and testing is not required for that stop. A longer course is tapered only once the disease that needed the tablet is controlled. The step-down aims at a physiologic daily equivalent, written as 4 to 6 mg prednisone, 3 to 5 mg methylprednisolone, or 15 to 25 mg hydrocortisone. Mayo Clinic's prednisone-withdrawal note lists irritability, lightheadedness, loss of appetite, nausea, joint pain, body aches, weakness, and severe fatigue if the cut is too fast. The ESE patient leaflet says a course shorter than a month can stop immediately; longer courses go down in written steps.

Steroid conversion: 5 mg prednisone equals 4 mg Medrol

Vanderbilt's conversion card (updated 25 August 2025) prints hydrocortisone 20 mg, cortisone acetate 25 mg, prednisone 5 mg, prednisolone 5 mg, methylprednisolone 4 mg, triamcinolone 4 mg, dexamethasone 0.75 mg, and betamethasone 0.6 mg as anti-inflammatory matches. Prednisone and prednisolone sit at 12 to 36 hours; dexamethasone at 36 to 72. LITFL uses the same 5-to-4 prednisone-to-methylprednisolone pair and the 4:1 hydrocortisone-to-prednisolone pair. Endotext's table keeps those milligrams and adds a biologic half-life of 18 to 36 hours for prednisone. Solu-Medrol is intravenous methylprednisolone on the same 4 mg line, not a second class.

NIDDK: sudden stop is the common tertiary cause

NIDDK writes that the most common cause of tertiary adrenal insufficiency is suddenly stopping corticosteroids after a long course. Prescription doses can sit above the body's own cortisol, so the hypothalamus makes less CRH and the adrenals go quiet. Symptoms on that page include long-lasting fatigue, muscle weakness, loss of appetite, weight loss, abdominal pain, nausea, vomiting, diarrhea, and blood pressure that falls on standing. The Endocrine Society's patient page adds severe nausea, vomiting, diarrhea, abdominal pain, dehydration, confusion, low blood pressure, and fainting as adrenal-crisis warnings. MedlinePlus lists sudden stop of a long glucocorticoid course among crisis triggers and wants intravenous or intramuscular hydrocortisone in that emergency.

Prednisone is a corticosteroid; albuterol is not

Mayo Clinic calls prednisone a corticosteroid, a cortisone-like steroid that works on the immune system to ease swelling, redness, itching, and allergic reactions. The same house files albuterol as an adrenergic bronchodilator: a short-acting beta agonist that opens bronchial tubes. ProAir and Ventolin are albuterol brands on that asthma page; they are not steroid inhalers. Oral prednisone or methylprednisolone may still sit beside a rescue inhaler for a severe asthma attack. Prednisolone is the same glucocorticoid milligram as prednisone on the Vanderbilt card. Eye drops and Orapred sit on that page.

DailyMed wants prednisone before 9 am, with food

DailyMed's prednisone tablet sheet says gastric irritation may fall if the tablet is taken before, during, or right after meals, or with food or milk. The adrenal cortex peaks between 2 am and 8 am and bottoms out between 4 pm and midnight. A single daily dose suppresses that axis least when it is given in the morning; the sheet recommends administration before 9 am. Large doses may add antacids between meals. Multiple daily doses are meant to be spaced evenly. Mayo's prednisone page repeats food or milk for the stomach.

Prednisone 20 mg is a peach scored tablet

One USP listing makes the 20 mg tablet peach, round, scored, debossed 5092 on one face and V on the other. Deltasone's 20 mg tablet is peach, round, scored, imprinted DELTASONE 20. The same Deltasone carton also lists 2.5, 5, 10, and 50 mg. Other USP strengths on those sheets are 1, 2.5, 5, 10, and 50 mg. The 20 mg search is that peach tablet, not a Medrol 4 mg ellipse.

US prednisone stays Rx only

DailyMed marks prednisone tablets as a human prescription drug, Rx only. Mayo Clinic says the medicine is available only with a doctor's prescription. Strengths on common USP bottles include 1, 2.5, 5, 10, 20, and 50 mg. A US pharmacy fills those tablets, or a Medrol Dosepak, after a licensed prescriber sends the script. There is no over-the-counter US prednisone tablet.
